Subscapularis

My PT identified it was tight and may have been contributing to shoulder pain. There's a lot of meat in that area and find it hard to get at with my hands the way she taught me.

No other therapists had ever looked at my subscap in the past. I think she only dug around in there because I mentioned my lats felt tight.
My two cents is to ask your PT why she thinks your subscap is tight. Muscle releases are good for symptom relief and to aid in rehab/recovery, but the tightness might not go away until you address why it's getting tight in the first place.

I don't have that answer for you, but it's likely to do with how you are moving, and what structures in your body aren't moving enough.

I feel like my PT and I have worked out a good plan and was only briefly in pain this time. Just need to stay on top of things.

I suspect all my internal rotators around my shoulders have been tight for 25+ years. There's a lifetime of poor movement and breathing I'm trying to undo. I'm sure many other factors such as repetitive jobs (tree planting, saw mill) and contact sports have contributed to my messed up body.
