Plausible hypothesis based on watching my grandmother clean the house in heels:

Maybe back then women wore heels so much they really could have performed the leg press in heels?
 
Even if the equipment was well designed and could be used effectively, such a video was going to be handled by the marketing department.

In other words, this video had nothing to do with what anyone did in the gym, only with the images that they thought would be effective in sales.

This still happens -- hence someone has to use the battle ropes in every gym commercial today.

Who are we guessing was the target buyer?
 
That validates my suspicion that you aren't personally acquainted with any buff, heel-wearing bikini physique competitors.

And hard to say what they were thinking back in the day. Until the 1980's the longest women's race in the Olympics was 1500m.
 
Sensible.

Marathons, for any gender, are one of the worst events to watch. Drags on for hours, endless shots from chase cars.

Doesn't even have cool crashes like cycling.
Or the egg-heads at the time were ( true story ) worried about the gals health and ability/capability.
