A trainer tells someone to stand over his back as he takes a plank. Is there anything fundamentally wrong with doing this as a practice? Would it cause flexion stress on the lumbar spine, or anything else? Does it make sense to you??

Ram, in theory, a strong and coordinated athlete can take it. In practice—what is the point?  If he wants a loaded plank, he should do heavy deadlifts or Zercher squats.

Un unloaded plank with a focus on max contraction is very valuable for developing feed-forward tension—essential in many athletic skills.
